We primarily provide maintenance treatment with buprenorphine products in a chronic disease care model for those struggling with opioid use. However, we can provide short-term detoxification using buprenorphine products over a period of 4-8 weeks. This treatment option is on a case-by-case basis. Some situations that may require this treatment include:
Reported or known misuse of treatment medications without access to alternative formulations.
Actively using or on other treatment medications and leaving to an area without access to ongoing care.
Actively using or on other treatment medications and facing incarceration without ongoing care.
Long-term treatment with pain medications with a dependence referred by a provider who is inexperienced with discontinuing narcotics and managing withdrawal symptoms.
Sunrise primarily uses generic Suboxone (buprenorphine/naloxone) tablets in our medication-assisted treatment program. As a partial-agonist, buprenorphine only activates opiate receptors to around 40% of their full potential. This is enough to eliminate withdrawal symptoms and help a person feel stable and normal. This also includes a "ceiling effect" when it comes to impacting the respiratory system. This means it's nearly impossible to overdose on this medication when taken as directed.
The naloxone in Suboxone is the chemical in Narcan, the opioid overdose reversal medication. When taken as directed, naloxone has no activity in the body. When misused, it can induce withdrawal symptoms. Given these features (as well as their long-acting properties), buprenorphine/naloxone is an ideal treatment medication.
Buprenorphine tablets without naloxone are only available in certain clinical situations, such as for pregnancy or when a patient has a documented allergic reaction to naloxone. A buprenorphine-only film is on the market (Belbuca).
For patients who struggle with self-administering their treatment medications, long-acting, buprenorphine-only formulations may be suitable for use. This treatment can also help individuals seeking ways to break their daily routine of self-administering anything.
Long-acting formulations may make it easier to taper off the medication at the end of treatment. Currently, the monthly injectable Sublocade and the 6-month implantable Probuphine are available. These medications are expensive and require prior authorization from insurance providers.
Vivitrol is a monthly injectable naltrexone that doesn't primarily treat opioid use disorder but the prevention of relapse following opioid detoxification. There are many challenges associated with this treatment in that various other non-narcotic medications have to be used to help a patient abstain from opiates for 3-14 days until such a time that the first dose can be administered. If given too soon, the naltrexone will send the patient into immediate and severe opioid withdrawal. For this reason, less than half the patients desiring this treatment make it to their first injection.
Half of the patients who receive their first injection typically discontinue treatment prior to receiving their second. For those that do make it to the second injection, success rates are similar to those treated with buprenorphine. Given the expense of this drug, prior authorization from insurance companies must be obtained.
Narcan contains naloxone, which can reverse the effects of opioid withdrawal. Narcan has no known side effects, drug interactions, or other actions in the body. It is safe to administer if someone is found unresponsive, and it won't hurt the person even if they are not experiencing an opioid overdose.
Given these chemical properties, everyone is encouraged to carry a Narcan kit. Sunrise provides Narcan to each of its new patients at intake.
